Before your appointment, new patients will be asked to complete forms that include medical history, demographic information, and insurance details. Any available diagnostic imaging, including X-rays, CT scans, or MRIs, may also be reviewed by your doctor to assist in developing an accurate diagnosis and personalized treatment plan.
